ELIGIBILITY For Residency At paladin Assisted Living

Paladin is the right choice for someone who:

  • Needs Assistance with medications, supervision of self-medication, or administration of medication
  • Needs nighttime assistance 
  • Needs assistance with meals, which may include feeding
  • Needs assistance with bathing, dressing, grooming, and toileting
  • Has incontinence issues
  • Exhibit's symptoms of mental or emotional disturbance, but is not considered at risk of imminent harm to self or others
  • Has Alzheimer’s, Dementia, or other memory loss
  • Needs assistance with mobility 
  • Is hearing impaired or speech impaired
  • Has a feeding tube, catheter, ostomy bag, uses oxygen, nebulizer, diabetic, needs therapeutic diet or other medical needs
  • Needs redirection minimally or frequently 
  • Uses a Hoyer lift or other transfer device
  • Is on Hospice
  • Needs short-term or respite stay
  • Has been recently released from a hospital, rehab, or nursing facility
  • Falls frequently or may be at risk for falls 
  • Is currently staying at a facility that has not or cannot meet their needs any longer
  • Experiences depression, anxiety, or loneliness
  • Needs daily activity, companionship, or more social interaction
  • Enjoys nature, animals, or water





Eligibility Disqualifications

  • Is permanently bedfast
  • Is a risk of imminent harm to self or others
  • Is a registered sex offender 
  • Frequently elopes
  • Exhibits abusive language or behavior
